About CDS

Centralia College offers support services for students with disabilities to help you access programs and services. We are here to help you succeed. If you are a student or prospective student with a disability, this Web page will try to answer your questions.

If you have a documented disability, with 3 steps you are well on your way to getting the help you need.

1. Apply for services by filling out the Request for Services Form (pdf).
2. Provide appropriate medical documentation from a medical professional. (Fill out the Consent Form (pdf) and take to a medical professional.)
3. Complete an intake interview either by appointment thru our office if you are/or will be attending classes on campus or by phone if you are enrolled in an on-line course of study and cannot make it to our office.
